Q: Which substance causes most car accidents?
F: Alcohol.
A: Hay fever remedies.

Notes: “More car accidents are caused by first-generation antihistamines, which are profoundly sedating, than by alcohol,” says Prof Stephen Durham, professor of allergy and respiratory medicine at the Royal Brompton Hospital, London. Second-generation antihistamines don’t cause such problems.

That's a pretty loose definition of a 'substance', though.
